go to this website: http://www.rncentral.com/careplans/contents.html and click on the care plan for "knowledge deficit". you will see that it has a column headed "plan and outcome". this will give you an idea of how to list a goal. that is only a general form. you should make your statements a little more specific for a school nursing care plan. they need to be positive actions that the patient is going to be able to perform and that you will be able to observe. in a nursing home your teaching plan for someone you have on a bladder training program for incontinency could have a goal that can be as simple as "patient will ring call bell for ambulatory assistance every time he needs to toilet".

here is another site that has several actual student care plans posted. some are quite lengthy. i didn't look at them so i don't know if there are any teaching goals in them, so i'll leave it for you to check out.
