Nursing DX help

I am really frustrated.....I am trying to come up with two more nursing diagnosis because my teacher requires me to have 10!!!!! This is on a healthy newborn this is what I have so far.

Disturbed sleeping pattern RT new environment

Healthseeking behaviors RT need for teaching on newborn cares and needs

Risk for ineffective thermoregulation RT decreased subcutaneous fat

Risk for infection RT umbilical cord stump

Risk for Aspiration RT immature gag reflex

Risk for injury RT sole dependance on one caregiver

Risk for fluid volume def. RT extremes of age

Risk for Imbalance Nutrition less than RT def. knowledge of parent

Can anyone think of one that they used for a healthy newborn???

Thanks in advance

disturbed sleeping pattern rt new environment

healthseeking behaviors rt need for teaching on newborn cares and needs

risk for ineffective thermoregulation rt decreased subcutaneous fat, r/t ineffective regulation secondary to extremes of age (newborn)

risk for infection rt umbilical cord stump, r/t increased vulnerability of infant secondary to: lack of maternal antibodies/lack of normal flora/open wounds (umbilical cord, circumcision)/immature immune system/ lack of immunization

risk for aspiration rt immature gag reflex

risk for injury rt sole dependance on one caregiver

risk for fluid volume def. rt extremes of age (neonatal transition)

risk for imbalance nutrition less than rt def. knowledge of parent

pain r/t the birthing process or circumcision.... that's the only one i have that you didn't have already. i'll keep thinking of more though.

instead of saying decreased sub fat say, immature compensation for changes in environmental temperature.

im doing the same thing but i only have to do three...lol. but im not sure about the risk of aspiration r/t to immature gag reflex. The healthy full term baby should have this reflex developed. Also, instead of extremes of age, I would put something r/t kidney function. Newborns have take a while before their kidney's can concentrate urine. If the baby were on IV fluids for some reason they can have fuild volume overload. i dont know. risk for fluid volume def r/t insensible water loss by large surface area and rapid res rate. Also at risk for jaundice r/t destruction of erthrpcytes created by a new increased oxygen filled environment.(or something like that)

Do you have Spark's & Taylor's Nursing Diagnosis Reference Manual? It is 10 times better than the Nurse's Pocket Guide...It has a ton of nursing care plans in it and has sections for adults, adolescents, children, mother/ neonate, geriatric, and wellness. It's about $35 on amazon, and it is 100% worth it, it is making nursing school a lot easier...

also, how did they breast feed? you could effective/ ineffective breast feeding
