My nursing care plan is as follows:

Self-Care Deficit r/t immobility s/t cognitive impairment AEB: Impaired ability to put on or take off clothing; Unable to groom self satisfactorily.

My Expected Outcome is: Patient will have few if any complications (i.e: agitation/anxiety) while being assisted by staff w/self-care needs.

I am having trouble coming up with and Outcome Criteria for this patient. She has severe dementia. Can you help?
