Nursing Analysis

I am having trouble with linking the following NSG Needs into an analysis form.

Primary Diagnosis is C-diff

Need or physiological Safety because of flexiseal, IV

Need for Hydration due to diahrrea

Need for Hygiene/Skn/Tissue Integrity

How do I link them all together?

    Do you mean help with a nursing diagnosis?

    You have identified three problem areas based on your assessment, now give a nursing diagnosis. You have 2 actual problems and 1 risk. While they all center around the medical diagnosis of c-diff they affect the patient differently as you have identified. Now think of three separate nursing diagnosis that would address those issues.
