Submitting direct care hours for a nursing practicum varies depending on your school or program, but the general process usually involves the following steps:

Track Your Hours

Use a tracking log or form provided by your school to record your direct care hours. This typically includes:

  • Date of service
  • Location or site
  • Patient or student demographic information
  • Activities performed
  • Number of hours per session

Get Your Preceptor's Approval

After you complete your direct care hours,  your preceptor (the supervising nurse or educator) must verify that they were completed.

Some programs may require your preceptor to sign off on your hours either physically on a paper form or electronically if your program uses an online system.

Complete Documentation

Ensure you've completed all necessary paperwork. This may include:

  • A reflection or journal of your experience
  • Documentation of competencies or skills you worked on
  • Evaluations from your preceptor

Submit to Your Program Coordinator

Once everything is verified and documented, submit your hours to your practicum coordinator or program director. This could be done:

  • Through an online system (like Typhon or Medatrax)
  • Via email if the forms are electronic
  • By hand if your school requires physical submission

Meet Submission Deadlines

Make sure you know the submission deadlines. Some programs may have weekly or monthly submission requirements.
