I did a care plan for "Readiness for Enhanced Self-Care", but I can't figure out what to use as an r/t... My NANDA book only says "to be developed" for the r/t factors... Could I use "an expressed interest to become more independent AEB: request made to Discharge planner, OT and PT?" R/t factors and AEBs have a tendency to confuse me..
Info on the pt: Recent stroke, but regained function on affected side... Independent in all ADLs, but at one point needed assistance dressing the affected side of her body...

"an expressed interest to become more independent" sounds like subjective data collected from the patient making it more of an AEB item.
The wording to use for the "related to" part is
These wellness diagnoses are all about the patient wanting to improve on what they already know/have.
