Need Help With Priority Nursing Dx for Comprehensive Care Plan

Background info: 16yr male /c WPW syndrome who came in for an electrophysiology study (EPS) and catheter ablation procedure. An EKG was performed at random, showing tachyarrhythmia and delta wave (why pt and family elected to do procedure). No previous surgeries. No indications of HTN, DM, CHF or other existing heart diseases/condtions. Pt healthy and athletic. Vitals were normal pre- and post-procedure. No pain pre-op (0 on scale). Mild pain (2 out of 10) post-procedure in mid to low back, non-radiating, hurt to breathe in. Pt had hx of chest pain and headaches. Pt was anxious immediately before procedure. General anesthesia was used; procedure lasted about two hours. Discharged same day.

How would you select prioritized nursing dx for a pt who otherwise would be considered healthy? Would they mostly be "risk for" dx and would that make them any less important? It still is a "plan of care" no matter what sort of plan it is, right?

I need to have three care plans for this pt. So far, I have two dx that seem to fit /c this pt condition:

1. Anxiety r/t impending procedure & deficient knowledge of the effects of general anesthesia

2. Risk for ineffective breathing pattern r/t CNS depression 2nd to anesthesia administration & temporary immobility (bedrest ordered q4hr post-op, keeping legs straight & together)

3. ???

think about complications, what you would look for. this could be a "risk for..." nursing diagnosis. look in your nanda-i for it. patient teaching, too... .
