Need HELP in reapplication

i took my exam last exam last jan 13 and I answered 265 questions, twas brain draining literally!:cry: unfortunately I failed! :bugeyes: And now I'm really confuse about the reapplication process. By the way, I'm applying for California. I have tons of questions, and hopefully You can help me guys! ;)

1.) Are the "Request Reapply / Repeat Examination" form, and the $75 fee are all I need for reapplication?

2.) Do I need to wait 45 days before submitting the needed requirements? or I can submit all of the requirements and just wait 45 days before scheduling my exam?

3.) What does "Last exam applied for" on the "Request Reapply / Repeat Examination" form?

:redpinkheit will really means a lot to me if my questions will be answered!
