need help with Nursing Dx

Imbalanced Nutrition r/t poor food intake, mania, AEB pt having weight loss, and too distracted to eat.

Disturbed thought Processes r/t inablitily to carry on a converstation AEB all throughout his assessment it read "too Psychotic to assess, flight of ideas.

Ineffective Coping r/t his breakdown, AEB psychotic break following rejection of marriage proposal.

I am a first semester nursing student and just trying to tread water, but I have to come up with nursing dx for a bipolar man with mania, and he is so overmedicated, he can hardly walk, talk, or do anything. When he is off his meds he is manic, and totally happy and wants to fix everything. This pt was found on the interstate, saying to himself he shouldn't have listened to the barking dog.

I just want to know if these are at least on the right track ...any insight would be helpful. I take criticism well and learn from it. :)

    The nursing diagnosis are fine, I think it is the related to's that need some work.

    When you write the related to, try to think of what is causing the nursing diagnosis.

    For example: ineffective coping r/t rejection (of marriage proposal) AEB psychotic episode (or something client stated ie; feelings of worthlessness)

    The ineffective coping is caused by (r/t) the rejection and you can tell because of (AEB) his psychosis

    In your diagnosis you say that his disturbed thought process is being caused by his breakdown, but really, the breakdown is a symptom of his ineffective coping.

    EX 2: disturbed thought process is not caused by his flight of ideas, it is caused by his psychosis (remember no medical diagnosis here). It is evidenced by his flight of ideas.

    Does this make sense? I am not suggesting that you use these as diagnosis', as they are not complete or accurate, but as a model. Try reworking them using this thought process and let me know what you come up with.

    I agree with above posters suggestions. Also I would suggest being more specific in the AEB part of your diagnosis. For example for disturbed thought process you state AEB 'states in chart' flight of ideas. Try being more specific like, "patient states..." or "patient displays..." You want to make it more patient centered and not as if you are citing another source. Do you have a good Nursing diagnosis book? I used "Nursing Diagnosis Handbook" by Ackley and Ludwig through nursing school and it saved my life! Good Luck to you :)
