Need advice/information sources for developing a wound assessment chart

Hi all!

I'm in the middle of a wound certificate and am stumped on am assignment which involves developing your own wound assessment chart. The rationale/justification for the inclusion or exclusion of each characterisitc should be included in the paper. wondering if someone could come to the rescue.

I am a registered nurse working in england in a care of the elderly home. I have been asked by my manager to put a wound care chart in place. Can you help please. There is no point cre-inventing the wheel don't you think. It would be much appreciated as you seem to have done some work on this.

Hi,

I think the skin intergrity of the client must be also be included in a wound assessment chart. This will indicate the baseline assessment of the client's skin intergrity and to anticipate how well the wound could heal. My instituition uses the braden scale scoring to assess skin intergrity of clients upon admission and we will constantly update the chart especially when our client undergo operation or any invasive procedure. Hope this will help.
