A nurse is caring for a client who is scheduled for amniocentesis. What information about the procedure should the nurse provide before the client signs the consent form?
1. Name of procedure, how its performed, description of alternate methods available, potential risk to mother and fetus, risks associated if the procedure isnt performed.
2. Name of procedure, risk to mother, name of physician who will perform procedure.
3. Name of procedure, risks to the fetus.
4. Description of alternate methods available, duration of the procedure, day and time the scheduled procedure will be performed.

I chose #4 based on the fact there is nothing in there about risks! The physician is the only one allowed to inform the patient about the risks of procedures right?
The correct answer according to NCLEX 4K is #1!!

Some of these books have horrible questions, #1 is more detailed and it explains everything but that's the physicians job. The correct answer would be to ask her if she had any questions and if yes to call the doctor back. I agree with your pick #4. Try a different question book if that one is giving you problems. What was the explanation for #1.

GOD I HOPE THERES NO QUESTIONS LIKE THIS ON THE NCLEX!!! YIKES!

There arent questions like that. The nclex is straight to the point, you either know it or you dont. Theres no trick questions.

Don't waste time worrying over this. When you get the questions, it will not take you long to decide how much time you want to spend mulling over any particular question. You are aware that you can't spend the entire allotted time trying to read into one question.

I would have picked #4 as well. We aren't supposed to teach about risks and stuff. Terrible question...
