New LTAC nurse and state has been called on my facility a ridiculous number of times. However this last time, they found a narcotic sign out sheet where my writing was not clear. It seems the count taken was 44 and I wrote 42 but rewrote 43 over it. The count was not off only the documentation. As it was unclear whether I wrote 42 or 43 originally. The number of pills in the blister pack for were incorrect not in the negative but the positive. As in there more present than accounted for. Is this an issue? I cannot believe this oversight was missed not only by myself but the oncoming nurse as well. Does anyone have any experience with this particular type of incident? What I should anticipate moving forward?

If there are more doses present than documented, at least it doesn’t look like somebody pocketed any. If the facility has been reported a lot of bad documentation or other errors, looks like this will add to their total. Be much more careful in your next job. Which you should start looking for right now.
