15 and 30 minute safety checks

I am looking for a 30 minute safety check sheet...basically one that shows the patient location plus a simple coding list for observation ..like sleeping, in group, watching TV. etc. We use a 15 minute one but it is not based on location but on observation of behaviors. anyone out there hwo would be willing to share?

We use one that has columns for staff initials, pt. location, and a wider column for a description of pt. activity.

pt. location is coded

BR Bathroom

RR Room Resting

RA Room Awake

GR Group Room

DR Dining Room

AMB Ambulating

ADM Admit Room

15 minute checks include pts location, activity, time, and initials of person who is documenting. 30 minute checks include pts location, time, and initials. Locations also sorta include activities like RA - Room awake RB- Room asleep, TV - TV room, G - Group activity but for the 15 minute checks, it's more detailed. You actually write something in addition to using the abbreviations.
