Live Scan Help!!!

Hey fellow nurses and heathcare superheroes!

This is my first post on allnurses.com and I need your guy's help!

I currently reside in Iowa, and I am applying for my license of endorsement as an LPN to work in California. I am flying to California to get live scan of my fingerprints done, but there is a section on the live scan form that asks for an OCA#. I tried to call the California Board of Nursing multiple times, but "due to a high volume of calls" I was not able to talk to a person.

My question is:

On the live scan form, if I am from Iowa registering for California license of endorsement, what would I put in the section that says OCA#? Thank-you in advance!!!

The mail code? I'm looking at my live scan form (just went through this process) and the mail code is 13425.

I just submitted a reply with a picture of the section of the Live Scan form I was talking about on this post about 15 minutes ago, but the moderators are still screening the picture before it shows up on the post.

But the section I am inquiring about is Section 4 of the Live Scan form where it says:

"Your Number: Lic # "

(OCA No. (Agency Identifying No.)

Level of Service: DOJ FBI

If resubmission, list Original ATI

Number"

I guess I was wondering if I needed to put anything in the "Lic #" part.

I was also wondering what you put for "Driver's License #:" if we aren't California Residents and I'm assuming you applied for licensure of endorsement for LVN. Did you need transcripts sent from your nursing programs and schools to the BVNPT? Thank-you in advance!!! ;)

So sorry for the late response. The BVNPT website can tell you exactly what you need to send in. I believe I did need transcripts from my program.

In the OCA section of my Live Scan form, the agent who took my fingerprints wrote "ENDORSEMENT" in that section. You probably don't need to write it yourself - thr agent will fill it in.

In the driver's license section you just write your driver's license number and then in parentheses what state it is from. Example: 123456789 (NY).

Whoa! No need for apologies! This has been incredibly helpful, thank-you so much! I followed the guidelines on the License of Endorsement, and it didn't say anything about nursing transcripts, but I am glad to hear that it won't hurt if I did send them with my materials anyways. Did you ever hear about applying online through BreEZe?
