Link for the ideal assessment note?

Hi all. Just found you and joined today:) I'm a 2nd semester student in an ADN program.

Does anyone have a link to sample health assessment notes? I've written a few but I'm afraid they're not going to be up to par for my current clinical instructor. She's very particular and likes to humiliate you in front of all your fellow students when you don't do things just right. Thank goodness this rotation is almost over:uhoh3:
