• Mail the fingerprint card and associated fee to CJIS Central

  the fingerprint card to 6776 Reisterstown Road, Suite 102, Baltimore
  
  
  Maryland 21215.
  
  
• Please include a check or money order made out to "CJIS Central
  
  
  Repository". Fees are found on CJIS's website

2. Complete the application online at https://license.mdbon.org/nets

• a digital passport style photograph
  
  
• the 12 digit number from under the barcode on the purple fingerprint card or the tracking number on the LIvescan receipt
  
  
• a Visa or Mastercard credit or debit card ($100 for the permanent license and $40 if you need a temp)
  
  
• Active license from another US State
  
  

3. Original state of Nurse License Verification

• If your original state of licensure is on NurSys, do your verification here: www.nursys.com
  
• If it is not on this website, you need to print this document (http://mbon.maryland.gov/Documents/verificationform.pdf), fill out the top 1/3rd and contact the state you took your boards. Mail this form with payment and they will send it to us with the information we need to process your application.
  

***Foreign Educated Nurses*** (nurses who have not worked, as a nurse, in the US for five years) (For those nurses who have an advanced degree from a US college or university is exempt from this requirement, upon proof of degree, i.e copy of diploma)

4. A score of 3 is passing on the ILA OPI.
   
   
   A score of 26 is passing on the TOEFL iBT.
   
   
   The Board accepts the IELTS if administered on October 01, 2005, or later.
   
   
   A score of 7 on speaking, 6 on all other modules, and 6.5 overall is passing on the IELTS
