letters of recommendation samples?

Hi, like I have said before I am volunteering in the OR at the hospital here on base. I know that I will need some of the people I am working with to write letters of recommendation for when I apply to nursing school. I was talking with my lab partner on the phone as we were going over the muscles that will be on our test on Tuesday. After we discussed muscles for a while, we started talking about other stuff to do with applying for nursing school and she said that a lot of people don't know what to write in a letter of recommendation and that I should get a sample or two to show the people I will be asking. I was wondering if any of you would know where I could find something like that or even if I could have a copy of some of your letters that worked well for you that I could show as an example. Any suggestions would be appreciated. Thanks in advance for any help.

Our registrar's office had a form for requesting letters of recommendation that I found useful. On the form you put the reason you were requesting the letter, your GPA, organizations you belong to, activities you participated in, and specific skills or qualities you would like to have addressed in the letter. You filled in specific information the school or scholarship application was looking for in the blanks. I found it very useful in obtaining appropriate letters of recommendation for scholarships and jobs after graduation.

Try giving the person an outline with your basic information as well as the information/attributes the school is looking for in the letter that you would like them to address.
