Is this safe?

I was taught that if questioning compatability between IV fluids and a med to: put the fluids on hold, pinch/clamp above the port closest to the pt, flush with ns, push med, flush again, then restart fluids. Are these flushes enough to avoid an incompatiblity reaction?

This is an accepted standard. As long as you are flushing with 5-10ml NS, there shouldn't be any problem with compatability.

Whew, thinks for such a quick reply! I was reading the TPN thread an it really got me thinking. I use 10cc prefilled syringes. I do try to find out compatibility, but when BP is in 200's and md orders something stat and the fluids have a Dextrose in it. I know it will take me some time research, so I've been doing the pinch and push when time is a factor.

TPN should be going in it's own dedicated line. Therefore, you should have another line (or more) for other purposes.

Thanks, I know what you mean! The fluids with dextrose was the D5's. I would never put something in the same line(lumen) as TPN. Our hospital has very strict procedures with TPN, we hang it like blood with 2 nurses with checks and double checks. If I've learned anything about TPN is to take it very seriously! If I ever had a question about it I would go straight to my charge. Luckily I have great charges nurse's and coworkers that love teaching the new grad!!
