Ineffective Protection

hi, i am a 2nd semester nursing student and i need some help with my care plan. my pt is 84/f who was admitted bc of an elevated inr or 7.9. i seem to be stuck! my nursing diagnosis so far is ineffective protection r/t abnormal blood profile secondary to coumadin therapy aeb pt takes coumdain at home, pt has history of falls, pt comes in weekly for pt/inr.

now the part i need help on is my interventions and the raionales to go with them. i think i have an idea of how this is supposed to go but i am currently stuck :confused:.. and need some guidance.

the intervention i have so far is:

*maintain a safe environment to prevent pt from injury my rationale is since she uses a walker and already has a history of falls safe environment can prevent her from injury and from bleeding (since her blood is so thin right now)

someone please help
