Improving I&O documentation

I am on a shared governance committee at work and have been asked to research ways to improve documentation of I&O's on our med-surg floor. Fellow nurses - any original ideas? Any visuals you place in the patient's rooms or bathrooms? (I'm not talking about clipboards in the bathrooms).

I've thought of creating a board in the pantry that shows the amounts of fluids in different sized cups just as a visual reminder that on our floor I&O's are something to be charted and paid attention to...

Nurses are busy, so are aides. I'm trying to find a creative way to get patients and nurses and aides on the same page that doesn't create more work. Thanks for any suggestions!

Linda
