I Need Help Please

I Have Been Nursing For Just About A Year Now.. Well Yesterday Just Near The End Of My Shift I Wrote An Order For A Patient. It Was A Medical Clearance From The Primary Physician For A Peg Tube Placement. Well When We Endorse Report To The Night Shift Nurse We Also Do A Chart Check Together To Assure That All Orders Have Been Implemented Or What Is Pending Etc... Well When I Did The Chart Check On That Patient The Order I Had Written Was Not In The Chart. I'm Like Ok.... I Figured I Had Not Finished It So It Wasn't In The Chart. I Wrote A New Order Put It In The Chart Informed The Nurse In Report Etc.... I Viewed All My Other Charts (7 Patients Total) To Be Sure That I Had Not Mistakenly Placed In Another Patient's Chart. Well Today I Receive A Phone Call From Work Informing Me That A Physician Wants An Incident Report Filled Out Because My Order Had Found Its Way Into His Patient's Chart. What Is Wierd Is That It Was Not One Of My Patient's Room So I Don't Know How It Got In There.... Me And The Director Of The Floor, Well She Is New And We Really Don't Mix Well... What Do You Think Is Going To Happen? I Am Stressing Out....
