I am a second semester nursing school student and I have to write a care plan with three different NANDA nursing diagnosis and I can only think of one related to my patient. My patient is an elderly man that was hospitalized to have an epidural placed in his back because of debilitating back pain from a failed back surgery. I have a diagnosis of chronic pain r/t failed back surgery AEB altered ability to perform normal ADLs. Could someone help me with two other diagnosis and some interventions I could use with this patient. Thanks so much for your help.:)

You have very limited assessment data for this patient but a few more NDs that jump out to me are:
1. Self care deficit: bathing/hygiene OR dressing/grooming (specify which one) r/t pain
2. Activity intolerance r/t pain
3. Anxiety r/t stress, pain, or change in health status
4. Risk for infection r/t surgical procedure
