hypoglycemics as PRNs

I went to do a resumption today for a pt who had a stay in a snf. His hypoglycemic med was not on the discharge Med list. He said he is not taking it at home either. He was getting BSs around 70 and 80 and feeling symptomatic. States his blood sugars are between low 100s and 140s, but never over 150. How should I tell the MD signing orders? Include on Med list even tho not taking? Is there ever orders for glyburide PRN?

i have had patients that have orders to take their med if sugar is over a number determined by MD,,,i would not include it on the med list as patient is not taking it at present.....i would have the patient monitoring their sugar, watch it over like a week and notify the MD if it is staying 140 or so.
