Hi, I am inquiring anyones experience yet with hylenex for subcutanous fluid administration on mild to moderate dehydrated peds pt. Please elaborate on experience and buy in from staff and doctors with this concept. thanks

Hello there!

I used Hylenex for the first time(second person on our unit) about 2 months ago. Our peds ppl decided that it would be a great thing to use...Our staff however, including myself, were skeptical...it is like infiltrating a KID!...lol...a lot of use were nervous use it let me tell you. I missed the in service(I work night weekends)so i had to read the packet that was left for us and pray to GOD that it worked. The 7-year old female (going on 27yr old) had been admitted for dehydration and the ED had attempted something like 10 times to start and IV and draw labs on her. She came to the floor as my pt with an IV and I was told she was just HL before bringing her up. So when I went to add a T-connector(b.c the ED never does) the line wouldn't flush. I tried Heparin and Saline to get the now visible clot in the hub to dissolve, but I was unsuccessful. So...we had to restart the IV. Attempted X7( i know it sounding horrible at this point...and I felt bad for the little one, but she really needed an IV)...with out success, so reluctantly I went to the Resident on that night and suggested, although I had was nervous as all get out, the use of Hylenex.

I pulled out a bunch of intima needles to practice (with everyone watching) and we all had our own little in-service at the nurses station. The pharmacy(overnight we lose our pedi pharmacists) never had an in-service on Hylenex which made/makes the use of it very nerve-wrecking because I had to verify and re-verify the dosage and how to prepare it...(with what I Knew i had to tell them how it should be prepared), and to my knowledge the physicians are trying to buck the use of it. They too are a bit apprehensive.

The pt I had experienced no adverse effects and it worked just like it was supposed to. She had a little bubble in her leg where I started it and and was very happy to learn that once the line was started that she felt no discomfort and was able to sleep peacefully the rest of the night. Now for me, I was constantly checking on her and making sure there were no changes in the size of the bubble in her leg and no signs of true infiltration. First thing the next morning she had good veins and was able to have a regular PIV started.

So...long story short...It works even though it seems a bit awkward and incorrect!
