How to write this paper?

HELP PLEASE!

I'm being asked to write a paper (APA fromat) based on the interview of a RN. I have already interviewed the registered nurse but I can't understand how to go about writing this paper.

Here are some guidelines:

Use a narrative format to address the topics listed below. Follow APA guidelines for the title page and the body of the paper, however no abstract or reference page is needed, unless you cite an author. Each question area is worth 9 points. The remaining 10 points will be given for grammar, spelling, neatness, clarity and APA format.

Basically, I was given Q's to ask the nurse and now I must use the answers to write the paper in a narrative format.

Can anyone provide me an example?

How can I do this? Since no reference page is required, should I assume that I shouldn't use quotes in this paper?
