Help With Discharge Plan

hi i was doing my discharge plan on impaired home health maintnenance r/t cva and one of my interventions was home health eval to promote compliance with medication regimen is this far fetched or ok??? the other was client will practice rom to assist with adls every day after discharge.

ok on these rationales should it be something stated in your own words like rom exercises-increase muscle strength and so on and then cite the page or quoted directly from the textbook i am having problems with these
