help! web-based assessment tools?

Hi, I am a third year nursing student. I am working on a research project which is trying to find out if there is an internet site or program that offers a culturally sensitive assessment tool that could be used in Dr's offices or right at the bedside for us nurses. So far we haven't been too successful but if you have heard of anything or currently use something like a web-based assessment tool for a variety of cultures... please let me know. Thankyou so much.
