Help question about LTC

I had a new admission on Thanksgiving The patient was a little aggressive and upset because he's hard of hearing and having anxiety about being in a new facility The doctor ordered an Ativan PRN Q8 hours for anxiety I wrote up and fax the order to the pharmacy I also filled out the behavior sheets

I got reprimanded by my supervisor for not calling the family about the meds. He is alert oriented and able to make his needs known He does not have a healthcare proxy or durable power of attorney I was told I must always call the emergency contact in the chart and let them know about things like this The family rather for him to be on Ambien instead of Ativan The patient wants Ativan it is really normal procedure in Pennsylvania I have a New Jersey nursing license and recently transferred it to Pennsylvania so I'm confused They also requested that he wears a diaper and he is fully continent it and does not need one

It just kills me how administration just loves to write new employees up for the STUPIDEST things! If the resident is continent, why should he have to wear a diaper and lose more of his dignity? SHEESH! I have been working in a SNF for five weeks and I have been written up at least twice for several things that I was either not aware of or that I did not do ie was not "guilty" of. I figure I am about to lose my job when I go in on Wednesday to have the Supervisor "talk" to me. I have 39 yrs of experience, but then, I am a "Hospital" nurse and just plain don't understand the SNF mentality. To be written up because I forget to chart the skin checks is really detrimental to the patient, but it is no big deal for nursing homes to not have AED's for codes. I just dont' get it!!

So the family can do this

You and I both. I don't get it. The answer to everything is to write you up for everything that has to do with nothing. But the things that should make sense don't and no one seems to be worried about that (eg. AED For codes)
