Help please!

Hi all,

I'm a new nurse, less than a year and I am currently at my first job. I am considering resigning due to the fact that I was railroaded by my supervisor for an issue beyond my control. I have a patient who is sexually inappropriate, agitated and they were transferred from another facility for that reason. When the behavior began I documented constantly, notified family, MD, supervisors, social services, the works. Each time I was told just keep documenting. Well this patient tried to go into a females room and kiss her. That same shift the patient became agitated and tried to escape. After redirecting and 1on1, resorted to the prn medication. That didn't help either. At the time I also had another patient who was extremely ill. Anyways, I was notified after the fact about the patient going into the room and documented it. The other nurse didn't follow up and now the family is upset. I tried to plead my case and show documentation where this has been ongoing, but my account had been deleted and all of my notes gone! I'm frustrated because I don't know what to do. I know I messed up in between and should have done more and I'm so sorry about it. It just seems like it's being taken seriously finally only because a family is involved.

Nothing is gone forever, your notes exist SOMEWHERE. I'd be raising some holy he!! with someone in IT to find them.
