Help with PES

Hi Everyone!

I need to rank my PES by priority for nursing diagnoses. I am having a difficult time with the risk for vs. the actual nursing diagnosis that I have chosen. So, this is what I am thinking for priorities to assess:

1. Risk for shock

-due to obstruction of an abdominal organ and pt has had N/V x3 days

2. Risk for fluid vol deficiency

-pt is NPO and has again had N/V x3 days

3. Constipation

-pt has not had a BM for x3 days

SO, those are my thoughts. If anyone has any comments or suggestions I would greatly appreciate it! Thank you!!!

I think you can combine shock with the fluid volume deficit (as in what to monitor for).

What is the medical diagnosis, age, and any pertinent vital signs/labs ? :)
