HELP for the NCLEX???

I do have some questions...it seems like my resources have different views in this matter...please hlep!!! These might be included in the test...

1) Informed consnent.---A minor is to undergo suegery. the nurse notes that the child's parents are divorced and have joint legal custody. The informed consent has been signed by the father.

A) Should the nurse contact the mother to obtain consent? or

B) Should the nurse admnister the pre-op meds since consent from either parent is sufficient

2) Which is the priotiy for any client with sickle cell crisis?

A) Adequate hydration? , or

B) Oxygentaion???

Well, for the first question, what i know is that the father needs to be contacted too..but then, the Qtrainer said that consent from either parent is sufficient.

For the second one my answer was to administer oxygen (and then adminsiter fluids)...but the Qtrainer said fluids should be given first.

Even with joint custody, one is designated as a managing conservator I believe. That is the one that can sign the consents. I'm not sure about a joint conservatorship? Interesting question. On the sickle cell, I would say fluids because it will help circulation and if you don't have circulation of the RBCs, oxygen isn't going to help much. Circulation is the big issue because the sickled cells stick together and obstruct small vessels. Plus with the sickled cells, their oxygen carrying capacity is decreased due to the mutation of the heme molecule, so will added oxygen make a huge difference if there aren't enough normal RBCs to carry the O2? :twocents:

Oh, okay. thanks. Guess i got so caught up with the fact that hypoxemia causes sickling..0-0 Got t remember that fluids should be given first before O2...

I know each state will have their own regs re joint custody but my understanding if joint then either parent's can sign consent form.

Sickle cell is very debilitating especially in crisis. Fluids as mentioned is very important as is pain management
