Help with a diagnoses

hello! I have to write a care plan for my patient on post partum this week. There is no dad in the picture only the mom (my patient) and her girlfriend. so i figure i can write a pychosocial careplan about the baby boy but i just dont really know where to start. any help will be great! thanks!

you have provided no specific information for me to help you. diagnosis is based upon abnormal assessment data. assessment consists of:

  • a health history (review of systems)
  • performing a physical exam
  • assessing their adls (at minimum: bathing, dressing, mobility, eating, toileting, and grooming)
  • reviewing the pathophysiology, signs and symptoms and complications of their medical condition
  • reviewing the signs, symptoms and side effects of the medications they are taking
