Help with care plan

Hello,

I am trying to write a care plan for someone with a LUE wound. It was infected, gangrenous and necrotic. It has had an I & D done and now has pink granulation tissue. The pt admitted to doctors to injection heroin onto the skin and wound area prior to hospitalization. She also has edema on the affected arm.

I need some help finding nursing diagnoses. So far I have:

Impaired tissue integrity r/t infection AEB 15 cm x 8 cm wound on LUE

Impaired physical mobility r/t decreased muscle strength AEB unequal muscle mass and weakness of left arm.

Acute pain r/t wound AEB verbal reports of pain 8/10 and facial grimacing

For the drug abuse problem, should I use?

Ineffective health maintenance r/t inappropriate judgments AEB history of heroine abuse

What should I use for the edema problem?

Something related to circulation or perfusion?
