Help me with Braden Scale

I found that I dont have too much problem with completing the Oasis Comprehensive Nursing Assesment (more or less) except for the braden scale..How do I determine which homebound pt is at risk for for pressure ulcer,would a diabetic pt be classified automatically under mild risk for pressure ulcers due to neuropathy? Does homebound status automatically puts for the risk of pressure ulcers?

The Braden Scale is pretty straightforward, you assess the 6 factors and add up the score. Which part are you having difficulty with?

To my knowledge, diabetic or homebound status does not "automatically" put anyone at risk, it depends on the individual. It sounds like you are trying to make sure you don't miss any risk diagnoses, which is OK, but it doesn't sound like you are doing the evaluation component.
