Some people butterly first, then put tape across the needle. I put tape across then needle first, then butterly. In my experienc when it comes time to take the tape off, if the butterfly comes off first then the tape across the needle does a better job of keeping the needle in place. If the tape across comes off first, the butterfly does do as good a job of anchoring the needle. Too often I have accidentally dislodged needles because the butterfly didn't hold.
As for tape residue - I have found that plastic tape leaves more residue than paper tape. In the meantime I use the finger of my right hand to anchor the needle while I pull the tape off with my other hand. That way if if the needle wants to come off with the tape, my index finger gives the needle enough traction to resist being pulled out with the tape.

The dialysis company that I work for says that you must butterfly all AVFand AVG. To do this you should put a piece across the wings of the needle, place the butterfly cross and then finally another piece on top. If you have patients that sweat alot that can also play a role in why the tape loosens.

What types of tape and taping techniques does everyone use. We have noticed and increased incident of needle displacements during Rx and when discontinuing the other needle--due to tape residue sticking to gloves.
