GI bleeds

Recently it seems ERs do not want transfers from nursing homes for GI bleeds. Thoughts on how bigger cities can get these patients the care needed? I understand nursing homes can monitor labs but outside if a bleed is bad 10/10 transferring for a transfusion is the best. The EMTs tend to have bad attitudes about this and the hospital providers. I am not understanding why.
