Getting licensed with DUI

hello,

I have a nurse friend who is looking for some help/guidance. She got a DUI a little over 2 years ago. She completed all of the court ordered evaluations and classes. She did court supervision and the case was dismissed.

She was able to get her Iowa nursing license in Sept of 2024 by providing the evaluations and proof that she completed her classes.

She is now trying to get her Illinois license and idfpr is requesting a different evaluation and she is unable to find anyone to complete it. The evaluation has to be completed by either an addictionist physician or NP with specialty certification in addiction. Unfortunately most places only have certified addictions counselors.

The form that they want filled out doesn't seem to pertain to someone who had 1 DUI prior to becoming a nurse ( I am going to attach the form here)

has anyone had to complete this evaluation for a first time DUI offense and if so, who did the evaluation and how much did it cost?
