Failed my lvn in 2002

I was marry at that time and changed my last name. Im lost i do not know how to start i finished my Lvn classes in 2002 and just took one time my NCLEX and failed. I want to take the test again after all this years and i would like to know if i glhave to go back to school again for LVN or just review? So how do I changed my last name on my certificate?

    The BON in the state you wish to practice in has all those answers. Don't be surprised if too much time has passed and you are ineligible.
