I am an engineer in the fields of human factors/ergonomics, safety, and systems. I am working on an incident where a newborn in the NICU was mistakenly infused with breast milk through an IV setup that included an IV syringe pump. The supervising nurse has stated that the pump company represented the syringe pump as appropriate for enteral feeding and provided the NICU staff with training on how to use the syringe pump for enteral feeding. The pump company is denying they made the recommendation or provided the training. Furthermore, they are professing that their syringe pump, the Medfusion 2001, was not used in any other NICUs for enteral feeding.
I am looking for someone who could help me find information that identifies the Medfusion 2001 pump was in use for enteral feeding prior to February, 2006. The identification of specific NICUs would be even better.
I would appreciate it if you could direct me to sources (people, organizations, or literature) that would have this information or know where I could get the information.
Thank you.

If I am making unfounded assumptions, I apologize. But I suspect that this involves a lawsuit, and I would suggest that you contact hospital and NICU administrators directly, rather than solicit information from bedside nurses on a website. It's a bit unseemly.
