I am a 2nd career RN (first career in sales/marketing). Graduated from BSN program 5 years ago and spent one year in ED (hated it), 3 years working as Field RN for CHHA (loved the patients but charting into the night grew old) and am now in intake (desk job) for large volume health system in NY. I just sat for Aug CCM Exam and feel I did well on test (won't get results until 10/31). Just wondering if I have enough experience to work as Hospital CM? I review Referral Forms and d/c docs as Intake RN, feel very comfortable dealing with patients and families (from my home care as well as sales and marking experience) but really don't have a lot of experience in hospital setting (only one year in ED).
I would also welcome anyone's feedback on "a day in the life" of a hospital CM. I see the referrals from my intake position but don't have any idea about DME ordering, referrals to SAR or Long Term Care Facilities or dealing with MD's for Face to Faces or discharge plans....
