hello can I ask help in making a concise clinical report.

Situaion: A patient, Mr. Harford, is a psych patient. He is going to be transferred to the general ward after surgery. HIs circumstances require special attention from the nursing staff. Charge Nurse Blessy Par from the psych gives information to the nurses about their patient.

My report:

Mr. Harford, a patient from the psychological ward, is about to have his surgery next week. After the surgical procedure, he is to be transferred to the general ward and is requiring special attention.

Mr. Harford was first admitted to the psychological ward about three years ago due to his extreme manic episodes. Family was then concerned that he might try to harm himself. Since then, Mr. Harford's psychotic episodes manifested frequently and violently. Although, the violence is not directed to the people who are close to him and so, do not be afraid to nurse him. Despite his attempt to harm himself, Mr. Harford is believed not having any suicidal tendencies.-----------------------------------------------------------Blessy Par, RN
