hello all............my client has a diagnoses of

and bilaterally breast cancer

I'm not good at this

I must come up with 3 nursing diagnosis

3 stg

5 nursing interventions

5 rationales

I did very poorly in 1 semester with this.Can someone please help me get a start on this....any help would be greatful.

more data is needed on this patient, both objective and subjective before priority ND can be determined. If you can provide more info that would be helpful. Some general ND for breast cancer patients may include:

Anxiety

Knowledge Deficit

Pain (acute)

Body Image Disturbance

  • Experts

you must understand that a care plan is the documentation of your problem solving. you must follow the sequence of steps of the nursing process in writing a care plan. those steps are:

  1. assessment (collect data from medical record, do a physical assessment of the patient, look up information about your patient's medical diseases/conditions)
  2. determination of the patient's problem(s)/nursing diagnosis (make a list of the abnormal assessment data, match your abnormal assessment data to likely nursing diagnoses, decide on the nursing diagnosis to use)
  3. planning (write measurable goals/outcomes and nursing interventions)
  4. implementation (initiate the care plan)
  5. evaluation (determine if goals/outcomes have been met)

so, before you can do anything, you must gather all your assessment data. (see step #1).

if you still are in need of help with getting your care plan started, please post a list of your patient's symptoms after going through your assessment data and i will help you with moving on to step #2 and #3.

:nuke:I had a hard time with care plans as well. I bought the Mosbys Nursing Care Plan 6th edition as the recommendation of my clinical instructor. So far my care plan have been great.

You can take a little bit from one nursing diag and add to another (as long as they apply) with the help of the book.
