CHF/DM Guidelines or Pathways

I am wondering if any of you would have some resources to share on where I might be able to find some guidelines or simple paths. I am in the process of developing some tools for my staff to use, nurses who are not really knowledgeable about CHF, DM (appropriate labs to monitor, education to provide to the pt) but want them to be quick and easy, check list if at all possible.

Any suggestions or links would be greatly appreciated.
