cheat sheet for SOC

Hi does anyone use a single page cheat sheet to make sure you have all the info required for SOC. I hate to add another paper to the pile, but I thought I might be helped by trying to get everything on one page while in the home. It feels awkward paging through mountains of paper work while interviewing and assessing. If anyone has such a thing I would love to have it!

I had made one for myself many years ago. I found myself forgeting the same things over and over. Mine were the pharmacy you use and the phone number,religious preference,glasses and hearing aids and assistive devices and emergency contact person and number. So I would make my own based upon whatver charting format you are using. i made the cheat sheet at the time when we were doing Oasis......and I loved it. It saved me time b/c i did not have to call the pt and/or caregiver back or wait until a next day visit to gather the information. So what are you forgeting or perhaps you can make yourself a slimmed down version.

I use a cheat sheet also! 2 columns with those most forgotten questions...ie dentures, religion, pharmcy numbers, emergency contact, ht and wt, immunizations/screenings,last bowel movement; self care deficits, the diet the pt states that they are on; equipment in the home. The other half of the sheet is my assessment findings; vitals/wound measurements, etc.

I photocopy these sheets yellow so that they stand out from my other papers. I then refer to this sheet when I am inputting my info into the laptop....(I use my laptop during the visit to enter meds only and use my yellow sheet for all the other stuff. Otherwise, I find that I am focusing on my laptop and decreasing the effectiveness of bonding/developing trust with my patient)

I am also thinking about developing a quick check off form for teaching since our agency wants us to document teaching 2 things per visit. This way I can look at what I have left to instruct. (Our company doesn't utilize the computer program's function for this)
