Change the shift report sheets

I am new to this form and I read your former post on creating your own sheet for a change of shift but I still have not been able to do it on my computer I'm computer literate but if there's a way could anybody send me a template or at least post one on this site because I'm unable to download it the one that was there thank you so much you guys are awesome oh I meant this for long-term care facilities as I'm working in one now and thank you

    I would offer to send you mine, but Im ICU and LTC is way different.  Well, maybe I'll see if I can attach it. Nope.  Mine is simply a Word document with body systems on the left side, a few other words space/tabed to where I want them; I print it, and draw lines like school paper.  If you do it that way, you can edit and rename different versions.
