Case manager

I worked on an Assertive Community Treatment team for almost a year and did have a caseload for case management. I did nursing stuff like assessments, pouring meds, addressing somatic complaints and giving long acting ILs. Case management is more social work type stuff. If you can familiarize yourself with the social welfare agencies in your area, great. Also treatment planning. Case management is a lot of teaching basic living skills: how to keep up a wherever they live with cooking/cleaning/laundry, budgeting, grocery shopping, making sure they get all their entitlements, etc.

CM for BH is part of my job right now. We follow up on recent discharges from hospitals, make sure the members have a f/u scheduled with their psych, educate/help manage meds, and act as a community resource. Many of our members have more 'intense' case managers with a company we contract with, those case managers have more direct contact with members. They may take them to appts or help with ADL'S.
