Case management position

Hi, I'm a new lvn and I saw a position for case management. Can someone tell me what are some of the job description for case management? thanks.

usually case managers are RN's . they handle the discharge planning for the patient. such as where they will be discharged to .. home or ECF or SNF , or do they have family to take care of them. they work with the insurance companies to make sure the hospital stay is covered or meets critera to either stay in the hospital or go home.

i'm sure they do alot of stuff i don't know about. i have done some case management in the past but not alot.

Many of our discharge and case managers are LVNs. In fact, it's pretty much the only position in our hospital that uses an LVN. We just don't have them around.

Agree with prior post - arranges for health care needs post discharge such as home O2, medical equipment (liaison with suppliers/insurance), follow up care, STR, LTR, SNF, interfacility transfers and the like.

Very administrative in nature, not much if any, bedside care. Requires solid communication and organizational skills.

thats great !! we only hire RN's for case managers at our hospital.which is usually hired by the insurance company. i think LVN's would be great in the job...

it's sad because we don't hire LPN's at all anymore.. not even agency LPN's to cover for med/surg...... it's a sad trend ...

I was a major case manager for ten years but I handled mostly Workers Comp and liability injures. And my responsibilities were quite different. I managed the medical/surgical care and treatment, in and out patient, rehab, monitoring home health care, PT,OT, plus vocational evaluation and placement. I even arranged for building on of necessary items to an individual's home to ensure his well being. I hold a Masters, was certified by the state of Florida and was a Certified Vocational Counselor based on passing an exam.

case managers in our hospital do all the above plus fight with docs about what they can order based on diagnosis and what insurance companies allow. they often have to force docs to transfer patients from ICU's or SDU's to med. floor or discharge them, make them change patient statis to observation vs. full admit, finding patients rides home...........needed but not worth the hassle!
