Hello, so I am trying to figure out my careplan and thought I would ask yall!
My patient came in with diarrhea and n/v, she is dehydrated and is now showing signs of renal failure. The part I am struggling on is the R/T factor...
I was thinking my Diagnosis would be Deficient Fluid Volume r/t compromised regulatory mechanisms AMB nausea, vomiting, and diarrhea.
Does that sound right? I was thinking the compromised regulatory mechanisms would correlate with the renal failure...

R/T means "caused by." Renal failure didn't cause dehydration. What caused the dehydration?
And evidenced by is where you list the symptoms/assessments that told you this was a problem. What are the symptoms of fluid volume deficit? (hint: it's not n/v and diarrhea)
