care plans :$ Help?!?!

I am nearing the end of the first class in the LPN program. We wrote care plans for our clinical patients and one of my nursing diagnoses was Readiness for Enhanced Knowledge aeb patient explained cardiac diet and why it is good for her heart, and "I know I should walk more." I know the related factors are the etiology, but this diagnosis doesn't have any listed related factors. What do I do?

A wellness statement is a one part statement, there for there is no r/t or AEB statements.

For example, in the instance of your patient, you could use readiness for enhanced nutrition, which indicates that your patient is ready to learn about the benefits of a cardiac diet and what she needs to do in order to follow one.

In order to be able to use a wellness diagnosis your patient must be able to meet two criteria. 1) They must have a desire and be willing to enhance their well being through education and action and 2) They must have the physical and mental capabilities to do so.

You will then construct your goals and interventions along the way that will best educate your patient in obtaining that enhanced well being.
