Please Help.... Do Clinical Case Managers create Clinical Care Plans or Case Manager Plans??? I cant find this answer anywhere and I really need to know!?

Probably depends on the role of the Case Manager - if they are in the role of discharge planning or in care coordination it would likely be more associated with a Case Manager Plan, however if the Case Manager is responsible for coordinating the clinical care of the patient than it might be a Clinical Care plan.
Really it depends on how the roles are defined. Most of the time a clinical care plan that is related to overall care of a patient is by the medical team, i.e MD, NPs, PAs.
